docs(wiki): record the declarative-vs-prohibitive reading of issue constraints PR #1872 の brake 1 round 1 で決着した読みを Decision Structure entry として state-form で記録する。issue の constraints が述べる「対象ではない」は scope の 宣言であって禁止節ではなく、判定軸は同一ブロック内の禁止形（`触らないこと` / `書かないこと`）との対比である。Scope-exceed dialogue confirm の negative-constraint clause 腕は禁止形を持つ項に発火し、宣言のみの項には発火しない。 Scope-exceed dialogue confirm を扱う既存 entry は wiki 全体と index で確認したが 存在しないため、supersede / depend / conflict edge は宣言していない。 _Sidebar.md の行を同一 commit で追加。 #1876
