Update Itasca map for OSGeoLive GeoExt demo

This pull request contains the following non-breaking changes:

• Adds a GeoJSON OUTPUTFORMAT
• Allows all OWS services (so WFS can be used)
• Adds a PROJECTION block and sets gml_include_items on the airports layer to allow it to be served as a WFS

It also contains the following changes that may affect other demos/use of the Itasca map. These are currently being reviewed on OSGeoLive:

• ctybdpy2 STATUS set to OFF (or it appears on all WMS services)
• IMAGEPATH set to "/var/www/html/tmp/" (as used on OSGeoLive)
• OWS_ONLINERESOURCE set to "http://localhost/cgi-bin/mapserv?map=/usr/local/www/docs_maps/mapserver_demos/itasca/itasca.map&" (as on OSGeoLive)
