What's new: Fixed a critical double-paste bug on macOS where spoken text was inserted twice into the active prompt box.

🐛 Bug Fixes

• macOS: Fixed double-paste on dictation — Cmd+V was posted to two CoreGraphics event taps simultaneously (kCGHIDEventTap + kCGSessionEventTap), causing the transcribed text to appear twice. Now posts to kCGHIDEventTap only, which correctly propagates to the session level automatically.
