[PRMP-754] Allow create doc ref service to process other file types

Jira ticket: PRMP-754

Description

• added EHR snomed code
• added EHR_ATTACHMENTS snomed code
• accept EHR uploads as .pdf
• accept EHR_ATTACHMENTS upload as .zip
• create respective file configs for LG, EHR and attachments
• add enum of forbidden file type that would never be accepted
• add patient_ods / gp_ods matcher in create & update doc ref services
• remove some obsolete ARF code
