Changed name of file and added 'temporary' to the msg.

@frankieroberto I've made some changes to these error msgs. Let's review together later. Things to consider:

Added link to the guidance page where you can download the paper form.
Changed the msg so we're not making it sound like it's the user's fault.
Added link to paper form.
For now I've used the same msg as on our 500 error - pending conversation with devs about when we'd display this?

Amended some of the proposed error pages as follows:
If PDS lookup fails
If we cannot show any vaccination history
If we can only show some vaccination history
