Add connection-level fallback to MCP dynamic selection

• capture MCP connection metadata and expose it for prompts
• add a connection-level LLM fallback that consults low-cost then primary models when tool filtering returns empty
• document the expanded mcpdynamic flow and new fallback behaviour
