Combining *.sql into merged patches

Contents:

1. Changed file formatting (added empty new lines at the beginning and end of each *.sql file);
2. Added USE perpetuumsa where necessary at the beginning of the file;
3. Update 11_Add_Presents_to_Hersh_market.sql with @itemDefinition ;
4. Combined individual *.sql files into patches using the command: cat *.sql > ../live_patch_3x.sql;
5. Added link to p34 / p35 in apply_all.bat .
