What is the current behavior?

The Request Mentor button is unresponsive.

It should submit the mentorship request form

Temporary fix - when clicking on 'Request Help' in the top nav, we can direct the user to the older, original Airtable form that still works and populates to the Airtable Mentor Request form: http://op.co.de/mentor-request / https://airtable.com/shrPzqty0aXtfWuuq

I tested out that form and it correctly populated my mentor request to the Airtable 'Mentor Requests' sheet, and notified the #mentors-internal Slack channel ~30 minutes later.

In the meantime, we can figure out why the current page (https://operationcode.org/mentor-request) and the back-end aren't populating the requests to Airtable, and also work on moving mentor requests to Discourse.

Upon success we used to redirect to /thanks which does not exist. Instead I show a success message.

#835 removed mentor request form while we discuss the best way to go about mentor requests by routing all requests to the airtable. Does this need to be open?
