[Bug] Long-session sudden crash

What happened?

A long session (2days) crashed on me with the following error:

Node.js v24.13.0

At the time of the crash, I was not doing anything specific, and the thread had been running by itself for over two hours since my last message.

I asked codex to help me get back in the thread, and it claims it was an agent that didn't shutdown cleanly. With its help I forced its shutdown and was able to recover the session.
