Updates the legal guards reference and changelog to reflect the fail-closed changes in qwed-ai/qwed-legal#25.

Changes

• legal/guards.mdx — Added a "Fail-closed behavior" section to StatuteOfLimitationsGuard explaining exact-match jurisdiction lookup, unverifiable results for unknown jurisdictions/claim types, and a worked example. Added a StatuteResult fields section documenting the new Optional types and the jurisdiction_matched / claim_type_matched flags. Updated get_limitation_period() and compare_jurisdictions() examples to show None returns for unknowns.
• changelog.mdx — Added an entry summarizing the fail-closed behavior, breaking changes (exact matching, Optional return types, removed defaults), and linking to the updated guard reference.
