Fix RDMA QP load imbalance

• Rotate RDMA batch starting EP by transfer id to avoid single-WR traffic always using QP0.
• Preserve QP-to-worker affinity in the multithreaded executor.
• Clamp max SGE handling and warn on short SQ backoff timeouts.
