- Added singleton-like RcmlserverResolver class and respective unit test. - Hooked voice, sms and ussd entrypoints. Refers #2340

This PR will add relative-to-absolute url resolving for urls that point to RVD applications i.e. resolves Application.rcmlUrl that start with '/restcomm-rvd/'. It will use restcomm.xml/rcmlserver configuration section to do that. Automated unit tests are also included and manual testing has been done for voice calls.
