[codex] Add remove in-use toasts

Summary

• show specific toasts when secret or connection removal is blocked because the item is still in use
• switch remove mutations to promiseExit so typed Effect failures can be handled directly
• add optimistic secret removal via Atom.optimisticFn so secret rows disappear immediately and roll back on failure

• bun run --cwd packages/react typecheck
