docs(word): close single-flight report and update decisions index

Summary

single-flight 관련 마무리 내용을 미션 톤이 아닌 보고서 톤으로 문서화하고 decisions 인덱스를 갱신합니다.

Problem

single-flight 안정화 작업 결과와 미적용 판단(특히 fencing token)을 삭제 전 기준으로 추적 가능한 문서에 남길 필요가 있었습니다.

Solution

• single-flight 분산 안정화 보고서 문서 신규 추가
• Redlock 적용, 폴백 경로, 검증 근거(PR feat(word): harden distributed single-flight with redisson and redis client unification #328/fix(word): patch single-flight lock expiry and follower error semantics #330/feat(word): Redlock 기반 single-flight 적용 및 검증 #331) 정리
• fencing token 미적용 사유를 "AI 요청 수 절감 목표에는 직접 효과 없음" 기준으로 명시
• decisions README 인덱스 업데이트
