Upstreams table formatting issue

For multiple upstreams the weight and status cells are not formatted in one column:

Fixed. The issue was printf %b counting ANSI escape codes as characters, throwing off the column width.

Changed the address and status columns: address is now plain text with %-28s (no ANSI in the padding calculation), status uses echo -e after the printf so ANSI codes don't affect alignment.

Run mtproxymax update.
