[Inject Error] User Injects Submissions Disappearing or being Misplaced

When users submitted injects, the submitted documents would appear under other injects or would not appear on screen as being submitted. The user files would appear on the backend but made the user end unusable

Occasionally the inject submissions would also appear under the wrong number on the backend for the graders
