Content pages: various improvements

This is a continuation of #188 and #184.

1. On each markdown-generated content page, all entities (categories, properties, implications) are listed that reference the given content page. (This was already a feature of lemma pages before.)
2. Document usage of content pages in contribution guidelines.
3. Add the suggestion form on content pages.
