Introduce personalNotes on day entries for private journaling while keeping clinician-facing notes for PDF export. Persist in the database, wire through backup and import, and add UI, localization, and tests. Update phase UAT and planning notes. Co-Authored-By: Claude <noreply@anthropic.com>
