Changes

• Removed 'historical files' text from intro paragraph to registration form
• Disabled 'Provider' file type checkbox under entities
• Set initial value for 'Provider' file type to True
• Set 'Provider' file type in registration entity write DB function to True and removed field from update db function
• Updated all instances of 'Plan Type' to 'Payor Type' (plus related vars)

Testing

1. On registration form, confirm intro paragraph reads: "This form should be completed and submitted to register as a data submitter. Please review the Data Submission Guide for details about completing and submitting this form, paying special attention to the schedule of submissions including test files and monthly files."
2. Fill out form and confirm 'Provider' checkbox under entities in unclickable and checked by default (for any number of entity entries) and that 'Plan Types' etc. now read 'Payor Types' etc.
3. Submit form and confirm in registrations listing 'View' modal that 'Provider' is shown under list of File Types and 'Edit' modal's 'Provider' checkbox behaves the same as that of the main registration form and that 'Payor Type' shows correctly
