chore: remove handoff artifacts from repo

Summary

• remove committed command-output handoff material from the AzureFox product repo
• remove the repo-only generator script and test that only existed to support that handoff data
• add an ignore guardrail for command-output and clear local artifact directories from the checkout boundary

Validation

• pre-push guardrail suite: 237 passed, 2 deselected

Notes

• the generated handoff material was preserved outside the repo under /Users/cfarley/Documents/AzureFox-reference/command-output
• this cleanup does not change shipped AzureFox command behavior
