fix: ensure_database tries configured DB before postgres fallback#88

Instead of always connecting to 'postgres' to check if the target DB exists, try the configured database directly. Only falls back to 'postgres' + CREATE DATABASE if the direct connection fails. Fixes managed Postgres providers (Clever Cloud) that block connections to 'postgres'. Also passes ssl/ssl_options to all temp pools.
