chore: add GitHub collaboration workflow and PR CI

• Add issue templates for bug reports and feature requests
• Add a pull request template
• Add GitHub Actions CI for pull requests
• Exclude generated/internal files from Biome checks to reduce noise
• Clean up remaining Biome warnings in the current worktree
