Deletes the CandidateAssessmentSupervisor Record. Sets SupervisorDelegates Removed date if no other CandidateAssessmentSupervisors exist with matching Supervisor Delegate ID. No confirm screen is included - should there be one?
NotificationSent added to SelfAssessmentSupervisor model and loaded to prevent spamming reminders.

Description
Completes the delegate nominate supervisor flow and manage supervisors view. Implements send reminder, confirm and reject supervisor functionality. Improves validation and markup of the add supervisor flow.
