Add capability-review PR outputs

Summary

• add the verifier capability_review projection and schema support
• render the new capability-review PR comment with top changes, trust-root warnings, required next steps, and artifact links
• add additive GitHub Action outputs for trigger/verifier/capability routing while preserving existing outputs and keeping decision as the preferred gate
• keep the legacy v1 findings comment available with pr_comment_style: findings
• document the rollout defaults and regenerate verifier schema plus llms-full.txt
