The project instructions require codex_changed_files.txt to contain both git status --short and git diff --stat (docs_build/dev/PROJECT_INSTRUCTIONS.md:864-866), but this regenerated artifact replaces them with only path/action lines. Any downstream reviewer or build gate that relies on the required status/stat evidence for this report-only run will see the artifact as non-compliant even though the report marks it PASS; please regenerate it from the required commands.

When this report is used as the OWNER decision record, the open-set snapshot is already stale: GitHub shows PR #159 was merged before PR #161 was marked ready, but this line says all five requested PRs remain open and the table still recommends closing #159 as superseded. That gives OWNER an impossible/outdated action for one of the audited PRs, so the GitHub-authoritative state and recommendations need to be refreshed.
