Summary

Replaced 8 recycled image references in upgrading-a-plan.md with dedicated per-concept images. Two existing images reused (account-menu.png, membership-change-button.png), six new placeholder PNGs generated (upgrade-picker, upgrade-summary, upgrade-no-change, product-upgrades-plan-group, product-upgrades-free, product-upgrades-premium). All references updated with refined alt text. Resolves #91, Ref #64.
