docs: Fix typos, update terminology, and add attribution in README#159

Analysis of the issue where Claude "breaks out" of DeepWork workflows
when users describe tasks with directive language ("Can you research...").
Key findings:
- This is a prompt interpretation problem, not deliberate escape
- User responses to workflow questions sound like commands
- Claude's instruction-following overrides architect mode context
Proposed solutions (phased approach):
- Phase 1: Reframe questions, add interpretation guardrails, self-checks
- Phase 2: Template-level guardrails if needed
- Phase 3: Sub-agent isolation architecture (if phases 1-2 insufficient)
