feat: add equlibrium score to CTSIB assessments

Calculate 2D correct Equlibrium scores (3D will come out with future releases)

• Add color styling to table if value exceeds boundary
• Show as a chart in the PDF as well (or enhance the table for spacing)
• Show comparison to age gender norm Display format on PDF  #189
• Simple illustration to show what pose we expect the patient in in Instructions (see [Condition] Guidelines #132)

The Condition ratios need to be updated to be derived from the equilibrium score, not time.

Merging this hefty dog without #132 and #192 since they are outside of the scope of the equilibrium score. These are the next two tasks I will work on after pushing a new iOS release for the team to test.
