• Reject managed-marker injection in ban titles or rules before writing CLAUDE.md / AGENTS.md.
• Normalize multiline or markdown-looking titles into stable headings.
• Update the init meta-rule fallback to use the GitHub-installable npx path.

Tests

• Expanded CLI coverage for idempotent updates, cold-start insertion, default target selection, global mode, invalid invocations, slash-command install output, and package/docs/CI contracts.
• CI now tests Node.js 18, 20, and 22.
