Phase 11 post-review hardening:
- runtime_run audit event for all dispatch paths
- mcp_run audit event for MCP tool execution results
- Input guard escalation: prompt (interactive) / deny (non-interactive)
- Fix missing import os in CLI commands (pre-existing)
- Fix UnboundLocalError in MCP runner except handler
- 50 new tests, 1481 total passing
