v0.1.1 — fix: % no longer disappears under rate limits

Fixes the 5h/weekly % vanishing when the usage API is rate-limited (HTTP 429). The value is now cached + persisted, the reset clock keeps ticking, and fetches back off instead of hammering. New setting: claudeQuota.usageApiIntervalSeconds (default 180). Install the attached .vsix via Extensions -> Install from VSIX.
