Reproduction steps

Enable CMCD with default or custom configuration factory, in HTTP query parameter mode (MODE_QUERY_PARAMETER)
Play any DASH content.

Expected result

CMCD data is sent as HTTP query parameter for manifest and media requests

Actual result

CMCD data is NOT sent as HTTP query parameter for manifest requests.
It is sent only for media (audio/video fragment) requests.

Reproducible on every DASH content.

Same fix is most probably applicable also for HLS (DefaultHlsPlaylistTracker.java) and SS (SsMediaSource.java).
