Question tool is way too eager

Description

I love the implementation of the question tool for streamlining back and forth, but I have to say it is way too eager. I will ask a deep, meaningful question and instead of answering or just having conversation with me the tool nearly always resorts to just asking me questions. The funny thing is, it will always followup questions with the answers I was looking for, but not until after I answer the question I basically just asked it.

I don't want to disable the tool as I find it convenient for that decision making stuff, but the prompt for when to use the question tool needs a lot of work. The tool is getting in the way of meaningful discussion.

Steps to reproduce

1. Ask literally almost anything insightful about an implementation idea you have to get the model's opinion
2. Be presented with questions that basically reask me the question I asked the model

• The model I use most often is 5.2-Codex. However this happens with Opus 4.5 as well.

• Consistent asking for {{plan}} #8333: Consistent asking for {{plan}} - shows repeated questioning pattern after answer is provided (similar to your experience)
• Task tool constantly used even after disabling it. #9379: Task tool constantly used even after disabling it - demonstrates the same "too eager" tool behavior pattern

Yes Ive noticed this too going to fix it hopefully in next release the openai models love the tool TOO MUCH

I appreciate that the latest release attempted to fix this, but its still just as bad. No matter what I ask codex 5.2 it still just throws back all questions with its own questions. I'm just going to have to disable the tool when working with Codex.
