In UpdateReadmeMojo, consolidate header generation to one method to generate all useful attributes. Remove unreliable attempts to lint asciidoc. Use attributes in body, and fail if the use is missing. Remove unused table generation mojos.

Use indexer extension to create tables and counts in index pages. Remove obsolete table generation mojos.
