Moving from 2.13.x to 2.20.x brought with it a host of API changes that needed to be documented in revapi.json. To keep things simple I cleaned out all the old entries; these entries don't need to be preserved indefinitely (as they only apply to compares of the current API vs. the previous one). We can then add in the exclusions needed for this change.

This PR replaces #2044

Note: much like #2052 this PR shouldn't be merged until CASSJAVA-122 is resolved.

CASSJAVA-122 is now fixed (thanks @lukasz-antoniak!), merging 4.x into this branch and kicking off another test run to confirm that we're good now.

Bah, I messed up the git history somehow so that this doesn't apply cleanly and can't easily be fixed by a rebase. To keep this simple I've created #2076 to contain the same fixes but with a clean history.
