Add two methodology guides referenced by SKILL-INDEX: - addon-test-runner-cadence-discipline-guide.md: long-running test runner status cadence, independent cadence triggers, no-progress updates, and recovery after missed cadence. - addon-test-dg-helper-completeness-guide.md: multi-gate helper design for asynchronous operations, unfakeable observable invariants, serial AND gates, and dry-run plus fresh-install validation. Also update SKILL-INDEX scenario navigation and detailed entries, and keep newly landed controller-crash, design-contract, PVC rebind, and ship-readiness entries.
