Passive capture fixes

• Whole-word redlines. A reconstructed change is now snapped to whole words, so the redline shows the real edited word (e.g. Background. → BACKGROUND-EDIT.) instead of a confusing mid-word split.
• Tidier output. changex seal now auto-removes the intermediate *.baseline.docx snapshot — leaving the original + .changex + the tracked .docx. New changex seal --clean keeps only the original + the tracked .docx (also drops the journal).

CI

• Bumped actions/checkout and actions/setup-node to v5 (silences the Node-20-actions deprecation).

Install / update: uv tool upgrade changex · pip install -U changex · then changex connect <app>
