Fixes 11 findings (3 CRIT, 3 HIGH, 3 MED, 2 LOW) from the v0.7.1
hotfix audit:

- CRIT-1: test file imports real parseVancouverForLive (no stale copy)
- CRIT-2: placeholder handling restored (Word Source Manager gap fix)
- CRIT-3: noVol regex handles dotted journal names
- HIGH-1: smoke test in npm run test:smoke + prepack
- HIGH-2: assertion checks unique signature tokens
- HIGH-3: smoke test cleanup hook (no leaked temp dirs)
- MED-1: stripTrailingParen strips all trailing parens
- MED-2: doiOnly requires symmetric parens
- MED-3: noVol limitation documented

Tests: 181/181 pass. Smoke test covers 6 b:Source entries incl. placeholder.
