The embedded REPL fails to start when open an individual file

The embedded REPL fails to start when you open a file outside a Leiningen project. Steps to reproduce:

1. Open VSCode.
2. Create a new file and save it with the ".clj" suffix.
3. The extension will try to start an embedded REPL instance but fails all the time.

Yeah it's probably because we try to start our nrepl process from the workspace rootpath, and it's probably undefined when dealing with single files.
We need to define a directory to work with when this happens. Any ideas @avli? Maybe the user's home directory?

Yes, using the home directory sounds good.

@avli Can you reproduce this issue after #50 fix? Because I'm unable to reproduce it.
