feat(map-efficient): add qualitative convergence evidence

Summary

• Add branch-scoped qualitative_convergence.json evidence for opt-in Monitor/self-review convergence loops
• Validate tail clean streaks deterministically, reject gaming inputs, and surface run-health metadata
• Document operator usage and update generated template trees
