FR: Add toggle to save plan filename without spaces (obsidian vault)

FR (feature request):
Add toggle to "save plan files without spaces in name" for Obsidian (at least).
Helps avoid issues in Claude Code CLI if used in Obsidian vault dir.

Replacement char can be any of: dash -, _ underscore. I guess these are the easiest.

The reason I personally stopped using spaces anymore for dirs or files names in my vault was obvious after starting to use Claude Code in my Obsidian vault's folder:

• There were too many issues with it forgetting to add the right escape char before space, losing tokens, money, and time to get the path right.

Addressed in #240.

Adding a filename separator setting in the Obsidian settings tab — dropdown with three options: Spaces (default), Dashes (-), Underscores (_). The live filename preview in settings reflects your choice.

Also hardened the title sanitization to strip additional characters (parentheses, brackets, etc.) that can cause issues in some environments.
