[FEATURE] submit_review tool for Opencode extension

A single slash command /plannotator-review limit the potential usefulness of review UI.

• Needs to be triggered manually, requires human intervention in the loop.
• Cannot submit committed changes
• Agent should be the one submitting the review not human.
