Display multiple UCs under Completed changes

Impacts Student, Institutions and Ministry view: Due to the fact that application can have multiple UCs reported on it, SIMS need to be able to accommodate displaying multiple UC records under Completed Changes in student application.

Acceptance Criteria:

• Display for students, institutions, and ministry all the UCs reported for an application under Completed Changes. Follow the current logic for displaying. Keep displaying for both Ministry and Student.

Current Behaviour
If multiple FT UCs are reported for FT application, only the first one is showing under completed changes.

QA:

1. Create 34 weeks FT offering and submit student FT application
2. Report FT UC for 17 weeks
3. Report another 5 weeks of UC for the same application.
4. Check Completed Changes: both events should be displayed with the most recent on top

Had discussion with devs, question is how we are going to display UC reversal in the table. Do we create a separate status just for UC or we ok with no entry at all and will be relying on notes.
