User story
As a user I want the content for the modified independent appeal to be updated for uploads section to be focused around the new attestation form so that the process is less intrusive. I also want the domestic violent ticket updated to have similar base content.

Upload supporting documents
Please provide all of the documents listed below:
- A completed attestation form from the student outlining the nature of the circumstances; and
- A letter from a professional third party confirming the student’s circumstances. A professional "third party" includes, but is not limited to: counselor, psychologist, religious advisor, doctor, teacher, police, and/or staff at a social service agency such as an organization supporting those experiencing abuse or violence.
Domestic violence exception updates
- FT & PT: 25/26, 26/27
- Replace the first line of the upload section
