Address List Editor - missing header and footer

Background:
The Address List Editor no longer has a header or footer as a result of the following error.

"Uncaught Reference Error: Unippear not defined"

Public facing BC Gov't sites should include a header and footer.

Task:
Restore the header and footer in the Address List Editor.
