need to think about it carefully because the way I currently generate the
Contents doesn't play well with Pandoc and the eBook conversion. Apologies,
more delay likely - I'm over my bad cold but about to hop on a plane for a
few days vacation (in NZ).

It all looks good but the line ## Chapter X: IPv6 Diagnostic Tools has to go. (I'll approve the PR but then remove it manually.)
If you want to change the section name from Tools that's a bit fiddly (see here). Let me know.
The ### subsections will not appear in the Contents. That's really a bug in the makeBook process but not easy to fix. I suspect the ePub version will see them.
First pass. Probably needs more proofreading.
