跟踪: opencode SSE stream hang 修复 PR #24762

## 修复内容

- provider.ts: chunkTimeout 默认值从 undefined 改为 60_000
- message-v2.ts: fromError() 新增 SSE read timed out 错误映射
- etry.test.ts: 新增 retryable + fromError 测试
