[codex] Split fingerprint package layout

Summary

• make .ghost/fingerprint/ the canonical portable package with a manifest and split prose, inventory, and composition layers
• move checks, intent, decisions, and cache into enforcement/, memory/, and sources/, then update package loading, scan, lint, verify, check, review, stack, and context-bundle emit paths
• update dogfood fingerprints, docs, skill bundle references, CLI manifest, changeset, and focused tests for the split package shape
