The code is now hosted under a different account, and uses a new name. This will fix various issues cropping up on e.g. MetaCPAN where (for instance) the GitHub repo can't be found.
The name of the dist has changed and hence the queue in RT has also changed, thus the URL to the RT bug page needed to be updated.

Update the GitHub and RT URLs to their current locations. Now services such as MetaCPAN can access the relevant data and redirect to the correct sites.
