Obsidian plugin review fixes.

• Remove the plugin name from the settings heading (the settings tab already shows it).
• Scope file access to the configured sync folder and drop whole-vault enumeration, so the plugin only touches paths inside its own folder.

Requires Obsidian 1.11.4 or later. Works on desktop and mobile.
